Add a comprehension quiz and consent statements
In the consent form's Settings, switch on Enable comprehension check and add questions with their correct answers. Under Consent statements, add tick-box or Yes/No statements, such as agreeing to sample storage, and click Save statements.
Where to find it
Consent form > Settings
Add a comprehension quiz
Under Comprehension Quiz, switch on Enable comprehension check.
Type a question, for example "What is the main purpose of this study?", and the Correct answer.
Click Add Question for each one, then Save Settings.
Participants answer before signing. A wrong answer shows "Incorrect. Please review the consent form and try again." Typed answers must match the correct answer; capital letters and extra spaces are ignored, so keep answers short.
Add consent statements
Under Consent statements, click Add statement.
Type the statement and choose Tick box - must be ticked to sign or Yes / No - participant answers Yes or No.
Reorder or remove statements as needed, then click Save statements.
This button is separate from Save Settings.
