Create an eConsent form
Open Forms > New Form > Consent. Upload your ethics-approved PDF, start from a template, generate a draft with AI, or build it from scratch. Edit the sections, set the signatures, then approve it so participants can sign.
Where to find it
Forms > New Form > Consent
Before you start
- You can edit consent forms: Study Owner, Study Manager or Administrator.
Choose how to create it
| Option | How it works |
|---|---|
| Upload Approved PDF | Fill Form Title, choose the PDF (up to 25 MB) and click Create Consent Form. Participants read the PDF and sign; the site countersigns. |
| From Template | Choose a template, such as basic clinical trial, observational study, pediatric assent, biobank, pre-screening, HIPAA authorization or re-consent, and review it. |
| Generate with AI | Choose the study type, describe the study, click Generate Consent Form and review every section. |
| From Scratch | Start with six standard sections: purpose, procedures, risks, benefits, confidentiality and right to withdraw. |
Finish with Create Consent Form. The new form is a draft in your study.
Edit the sections
Open the consent form and click Edit.
Fill Form Title and, if different, the title shown to participants.
Optionally click Import from Word (.docx) to replace the sections with your document, keeping bold text, bullets and tables.
Edit each section's heading and content. Use Bold, Highlight in yellow or Small print, and the arrows to reorder.
Click Update Consent Form.
Then set the signatures and options, preview it and approve it: the Study Owner clicks Approve and signs, as for other forms. Until approval, participants see "Consent form not available yet".
