Sign your consent form online

Read the consent form, answer any check questions, enter your name and email, tick the statements you agree to, sign with your finger or mouse (or type your name), and press Sign Consent and Continue. If the study offers it, you get a copy by email or as a download.

Sign the form

  1. Read the consent form carefully. If it is a document, press Open consent document to read.

  2. If there is a Comprehension Check, type your answers and press Check Answers.

If an answer is not right, read the form again and press Try Again.

  3. Under Your Information, enter your Full Name and Email Address (and phone number if asked).

  4. Answer each statement. Some must be ticked to sign; others you can answer Yes or No.

  5. Sign in the box with your finger or mouse, or type your full name if the form asks for that.

Press Clear to start your signature again.

  6. Press Sign Consent and Continue.

You see "Consent Signed". Your signature is stored with the date and time.

If someone signs on your behalf (a legally authorized representative) or a witness needs to sign, the form asks for their name and signature too.

Get a copy

If your study offers it, a copy is emailed to you, or you can press Download Consent PDF. Later, you can find it in your portal under Settings > My Documents. Your study team may also need to countersign the form; you may then get a second copy.

If the button does not work

  • Read the red message above the button: it says what is missing, such as your signature or a statement.
  • In the portal, the Sign Consent button stays grey until every required part is done.
  • "A different version of this consent form applies to you": reload the page and read the current version.
