Where to find it

Visit > Subject Visit form

Before you start

  • You are site staff with data-entry rights: Principal Investigator, Sub-Investigator or Study Coordinator.

Record the visit

  1. Open the visit from the Casebook and click Enter on the Subject Visit form.

  2. Answer Did this visit occur?.

  3. If Yes, enter the Visit date. If No, choose the Reason visit not done: missed visit, subject withdrew, lost to follow-up, not required per protocol or other.

  4. Click Submit.

The date appears next to the visit name everywhere, and the next visit opens.

A date outside the visit window shows an amber dot in the Casebook, and the study may raise a query asking you to document why. Visits counted from this one are re-dated from the date you enter.

When a visit did not happen

Answering No locks the other site forms of that visit ("Visit not done") and counts it as missed. Depending on the study set-up, visits that depend on it may stop. To enter data after all, change the Subject Visit answer back to Yes with a reason.

Why the next visit is locked

A visit opens once the previous visit's Subject Visit form is submitted. Most studies only remind you ("Previous visit not completed"); some block data entry until it is done. Unscheduled visits are never blocked.
