Set the form code (OID) and SDTM mapping
In the form editor, open Settings > Identity. Fill the OID (or click Suggest from name), pick the SDTM domain and Form role. Each question can carry its own SDTM variable in its display options. The OID is required before approval.
Where to find it
Form editor > Settings > Identity
Set the identity
Open the draft form and the Settings tab.
In Identity, fill OID (required to approve), or click Suggest from name.
Use capital letters, digits and underscores, starting with a letter. It saves as you type.
Pick the SDTM domain, or No standard domain.
Pick the Form role: assessment, procedure, eligibility, consent, randomization or visit admin.
For individual questions, set SDTM variable in the question's display options, or leave it blank for automatic mapping.
