Record a questionnaire licence
For a copyrighted patient questionnaire, open its Settings tab, switch on Licence required under Copyright & Licence, and fill the licence number, expiry date and holder. Click Save licence details.

Questionnaire editor > Settings > Copyright & Licence
Record the licence
Open the questionnaire's Settings tab and find Copyright & Licence.
Switch on Licence required.
Fill Approval / licence number and Licence expiry date, and optionally the Licence holder / source.
Click Save licence details.
What happens
- While the number or expiry date is missing, staff see a "copyright approval pending" watermark in the builder, preview and PDF. Nothing is blocked.
- 30 days before expiry, staff see an amber warning.
- After the expiry date, no new questionnaire tasks are created and new submissions are refused. Collected data stays visible and exportable.
