Create an eCRF or ePRO questionnaire
Open Forms, click New Form and choose CRF (site staff complete it at a visit) or ePRO (the participant completes it on their own device). Name it, click Continue to Add Questions, and build it. New forms start as drafts until they are approved.
Where to find it
Forms > New Form
Before you start
- You can build forms: Study Owner, Study Manager, Administrator or Translation Manager. Site staff and CRAs do not see the Forms menu.
Create a blank form
Open Forms from the left-hand menu and click New Form.
Choose CRF ("Site staff completes at visit") or ePRO ("Patient completes on their device"). For consent, see create a consent form.
Fill Questionnaire Name and Description, and pick the Language if it is not English.
Click Continue to Add Questions.
The form opens in the editor as Draft v1.0.
Click Add Question and build the form (question types).
Prefer a head start? Describe the form in Generate with AI on the same page, use Build with AI inside the editor (build a form with AI), or pick a ready-made form from the template library.
Copy an existing form
Click the copy icon on a form card ("Copy as new form"), change the Display name, give it a New form code (or click Suggest from name) and click Copy as new form. This makes a separate form. To change a form already in use, create a new version instead.
Before the form can collect data
- Set the form code (OID) in the form's Settings tab (form code and SDTM mapping).
- Preview it and test it in a draft study.
- Approve it: site data entry stays blocked until a CRF is approved.
- Add it to the visit schedule.
Questions
What is the difference between an eCRF and an ePRO?
An eCRF (case report form) is completed by site staff, usually at a visit. An ePRO is a questionnaire the participant completes on their own phone or computer. See ePRO in Capture and the eCRF builder.
Can I move a draft from ePRO to CRF?
Yes. Click the arrows icon on a draft form card ("Move to CRF" or "Move to ePRO"), as long as it is not locked by a live study.
